Getting the most from your plan is easy – and we want to make sure you understand exactly what you get from your benefits.

Your PCP’s name is on your member ID card.
"Number two. Work with a Care Manager to help you get access to the care you need.

If you have a condition like diabetes or high blood pressure, our Care Managers can help you understand your benefits and coordinate your care.
"Number three. Schedule your annual wellness exam. Getting the preventive care you need is key to staying healthy.

It’s so important for you – and to us - that we’ll send you reminders when you need to schedule important exams, screenings or vaccines, like the flu shot.
When you get one of these reminders, make sure to schedule your appointment right away.

You should also ask the office to let you know if they’re running late so you won’t have to wait too long when you arrive.

You can register and sign in at horizonNJhealth.com. Click “member sign in.”
Once you’re signed in, you’ll have ‘round the clock access to your plan information.
You can change your PCP, request a new member ID card, and enroll in a Disease Management Program.

Our Nurse Chat lets you connect with a registered nurse who can answer your questions. If you need to speak to a doctor, the nurse will set up a video call.
