Out of Town?

If you are out of town and find yourself in need of emergency medical care, you should go to the nearest hospital and remember to show them your Horizon NJ Health ID card.

However, any non-emergency visits to a doctor must be coordinated by your Primary Care Physician (PCP) before you receive care. If you are in need of medical attention that is not an emergency, call your doctor right away to get help in finding medical care in your area.

Horizon NJ Health will need to coordinate care between your doctor and any doctor not participating with Horizon NJ Health. Horizon NJ Health will not cover care received outside of the United States and its territories.
