Horizon NJ TotalCare (HMO D-SNP) has partnered with Valued Relationships, Inc. (VRI) to provide members with an easy-to-use in-home monitoring system to take his or her vital signs. VRI’s nurses will monitor the member’s readings, and if their vital signs are outside of the normal ranges, a nurse will reach out to the member to see how he or she is feeling. Nurses will communicate directly with the member’s Primary Care Provider to let them know how the member is doing and to keep the member out of the hospital.
