Precertification/Prior Authorization Requests Submission Process to Change:
Effective September 1, 2019 , Horizon NJ Health will no longer accept precertification/prior authorization of initial intake requests for Prior Authorization of services by fax. Requests for precertification/prior authorization will not be accepted through the following fax numbers on and after September 1, 2019 :

Beginning September 1, 2019 , requests for Precertification/Prior authorization must be submitted through CareAffiliate or by calling 1-800-682-9094 , Monday through Friday, between 8 a.m. and 5 p.m., Eastern Time.

- Select Horizon NJ Health .
- Mouse over Referrals and Authorization .
- Click CareAffiliate.
