The partnerships Horizon NJ Health has with community groups and other healthcare organizations help us take care of our members’ needs. Often this care goes beyond a member’s medical needs and touches the social needs as well. These partnerships strengthen our ability to serve our members.
The Division of Medical Assistance and Health Services (DMAHS) is the NJ State department that oversees the NJ FamilyCare and Medicaid Programs. Horizon NJ Health works closely with DMAHS to make sure that members are enrolled quickly and have access to all their offered benefits.
NJ FamilyCare is the State of New Jersey's way of providing low cost health coverage for kids and certain low-income parents. Horizon NJ Health is a participating health plan, offering benefits to NJ FamilyCare members.
The New Jersey Division of Aging and Community Services administers programs designed to make it easier for seniors to get the help they need to support their well-being and maintain themselves in the community for as long as possible with independence, dignity and choice.
The New Jersey Health Literacy Coalition serves as resource to help improve health literacy on behalf of individuals and families. Improving a person’s Health Literacy helps the person take charge of their health and to lower health care costs.
