Horizon NJ Health Physician Directories for each county in New Jersey are provided below.
The directories contain Primary Care Physicians (PCP), OB/Gyn, vision, hospitals and pharmacies.
Each directory also lists physicians for the surrounding counties.
If you need help finding a specialist, please call your PCP or our Member Services department
at 1-877-7NJ-HEALTH (1-877-765-4325). Hearing- and speech-impaired members may use their TDD/TTY
equipment or service to call our toll-free line at 1-800-654-5505.
