How Do I Renew?

In order to be a member with Horizon NJ Health, you have to keep your NJ FamilyCare eligibility. This is what you need to know and do:

You must fill out a Renewal Application EVERY year. Call NJ FamilyCare at 1-800-701-0710 (TTY 1-800-701-0720) to find out your renewal date or to ask for a renewal form. NJ FamilyCare members who are not Aged, Blind and Disabled (ABD) can fill out a renewal application online. Click HERE to complete your form. Please be sure to pay your premiums on time if you pay a premium.

All members can either contact their case worker at their County Welfare Agency (CWA) or call NJ FamilyCare at 1-800-701-0710 (TTY 1-800-701-0720). You can call the Managed Care hotline toll free at 1-800-356-1561 (TTY 711). A list of CWAs by county is provided below.

If you don’t renew your NJ FamilyCare eligibility, you might have to start over as a new applicant, and this approval process will take longer.

Open and respond to all mail from your CWA or your eligibility determining agency.
