First you’ll need to find a provider that’s in our network. To do that, you can search our online Provider Directory or call Member Services at 1-877-7NJ-HEALTH (1-877-765-4325). If you find a new doctor by using our provider directory, you’ll need to call Member Services so they can make the change. You will then get a new Horizon NJ Health ID card in the mail with your new doctor’s name and phone number.
