You must see a provider who is contracted with Horizon NJ Health. Each member is assigned a primary care provider (PCP). Your PCP will coordinate all of your health care needs. If you receive care from a provider who does not participate with Horizon NJ Health without our approval, you may have to pay for the care.
To know if a doctor or dentist is part of Horizon NJ Health’s network, check our Provider Directory or call Member Services toll free at 1-800-682-9090 (TTY/TDD 711). Our Provider Directory lists all Horizon NJ Health physicians, dentists, hospitals and specialists.
First, you’ll need to find a provider in our network. To do that, you can search our online Provider Directory or call Member Services at 1-800-682-9090 (TTY/TDD 711). If you find a new PCP using our Provider Directory, you’ll need to call Member Services so they can make the change. You will then get a new Horizon NJ Health member ID card in the mail with your new PCP’s name and phone number.
