How Horizon NJ Health Works

Your Primary Care Provider(PCP)

When you enroll with Horizon NJ Health, you choose a personal doctor or a nurse practitioner called a Primary Care Provider (PCP). Your PCP is the first source for your health care. Your PCP’s name and phone number are listed on the front of your Horizon NJ Health member ID card.

Visit Your PCP Horizon NJ Health covers PCP visits, like well-care check-ups and baseline physicals. You do not need to wait until you are sick to see your doctor. Your PCP will make sure you are getting the quality care you deserve. You can help by keeping all of your appointments.

Changing Your PCP You can change your Primary Care Provider, just call Member Services. You can search for a PCP by using the Physician Directory (in the right hand column) or you can ask Member Services for help.

  • Changes to your PCP will be effective within 10 days
  • You will get a new member ID card if you change your PCP
