Horizon NJ Health developed its Social Case Management Department to help members manage their complex medical
and social situations and overcome life's challenges. A team of state-certified, licensed social workers and outreach workers opens doors of access and opportunity to members in need.
Members are provided assistance with crises interventions and are linked to services for mental
health/substance abuse, community and government programs, or more resources, as needed, for their care.
Social Case Management ensures medical follow-up after hospital stays or emergency room visits and access to preventive health care services.
In addition, members receive help with problem-solving skills, empowerment, self-determination, and many issues and needs, including:
- Pregnancy, infant care, baby needs, and parenting assistance.
- Personal care attendants (home health aides).
- Mental-health and substance-abuse programs.
- Lead screenings.
- Immunizations.
- Asthma.
- HIV/AIDS.
- Special needs.
- Community programs.
- Housing programs.
- Advocacy.
- Food and clothing.
- Women, Infants, and Children (WIC).
- Medicaid and NJ FamilyCare.
- Domestic violence.
- Support groups.
- Transportation programs.
