| Selecting and Changing Your Primary Care Physician (PCP) and Dentist

Horizon NJ Health members can choose a personal doctor or a nurse practitioner, called a Primary Care Physician (PCP). If eligible for dental services, members also choose their own personal dentist. A member's PCP and dentist are the first sources for health care.
Visit the PCP and Dentist
Even when members are not sick, Horizon NJ Health covers PCP visits and eligible dental visits like well-care checkups and baseline physicals. Members do not need to wait until they are sick to see their PCP or dentist. The PCP or dentist will make sure members and members' families are getting the quality care they deserve. Members can help by keeping all of their appointments.
Specialists and Referral Forms
Specialists are available for members whose PCP or dentist says specialist care is needed. The PCP or dentist will make the arrangements to refer members to a specialist to get all the care they need.
- Members need a Referral Form to visit a specialist.
- Women do not need a Referral Form to visit their Ob/Gyn or to get a mammogram.
- Members who are eligible for dental services do not need a Referral Form for routine dental.
- Members do not need a Referral Form for routine eye care.
Changing Your PCP
Members can change their PCP at any time. Member Services will help members choose a new doctor or dentist. Call our Member Services Department toll free at 1-877-7NJ-HEALTH (1-877-765-4325). Hearing- and speech-impaired members may use their TDD/TTY equipment or service to call our TDD/TTY toll-free line at 1-800-684-5505.
