Children and parents may apply for NJ FamilyCare. To be eligible, you must be without
health insurance for at least three months. There are some exceptions to this rule, so
contact the Horizon NJ Health Outreach Center toll free at 1-800-637-2997 for more information.
Eligibility is based on the number of people in your family and the family's total income.
