We are pleased to announce that Horizon/Mercy has changed our name to Horizon NJ Health as of June 1, 2004. But ONLY our name is changing. Horizon NJ Health members will still get all the same benefits and services that they have come to expect from their health plan.
Members can still use all of the same doctors and health care providers that they have been using.
Horizon NJ Health will continue with our mission to make sure you and your family stay healthy.
New Plan Materials
Members can expect to receive new plan materials, which include our new name. When you receive your materials in the mail, you may want to take some time to review them.
New Member ID Cards
Horizon NJ Health members will receive new ID cards for each family member. The new cards replace the current Horizon/Mercy ID cards. When you receive your new ID cards in the mail, please check them to make sure that the information is correct. You can begin using your new Horizon NJ Health ID cards when you receive them.
New Member Services Number
If you have questions, our Member Services Department is available to take your call 24 hours a day, seven days a week. Please note that the Member Services phone number has changed to 1-877-7NJ-HEALTH (1-877-765-4325). This is still a toll-free call. Our TDD/TTY (1-800-684-5505) has not changed.
