| Diabetes Management Program

Horizon NJ Health has a special program designed for pediatric and adult members who have been diagnosed with
diabetes. The Diabetes Management Program can help members take control of their diabetes by learning
about the disease and the long-term effects diabetes can have on their health.
The Diabetes Management Program also provides diabetic educational materials on topics, such as meal
planning, insulin, and medication use, and it assists members with finding a diabetic specialist and/or
nutritionist. The program also teaches members how to better manage their diabetes by assisting them with
obtaining a glucometer and supplies with a prescription from their Primary Care Physician (PCP).
Members who are enrolled in this program can get prescriptions for insulin and syringes from their Horizon
NJ Health PCP.
