CHAMPS — Health and Wellness Program for Children and Adolescents

Horizon NJ Health has designed a program to help maintain the health of our young members. The CHAMPS Program helps keep members' immunizations and well-child visits on track from birth and all the way up until they are 21 years old. CHAMPS stands for Children's Health Assessment and Maintenance of Preventive Services.

It is important that children get all the immunizations and well-child visits they need to help them stay healthy. Many diseases can be avoided with the proper immunization and/or regular checkups.

The CHAMPS Program can assist parents and guardians with scheduling doctor appointments and providing information on such topics as lead poisoning and child safety.

Members who enroll in the CHAMPS Program will receive help with the following benefits and services:

  • Immunizations that protect from serious illnesses, such as mumps, polio, and hepatitis B.
  • Access to outreach social workers who can help with personal needs or concerns.
  • Access to Women, Infants, and Children (WIC) services.
  • Access to pediatric specialists.
  • Durable Medical Equipment (DME) services.
  • Involvement in meeting Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) standards.
