Asthma "Breathe Easy" Program

Horizon NJ Health has an asthma program designed to help members reduce the risk of asthma attacks, relieve attacks when they do happen, and educate members about how to prevent asthma attacks. The program was developed using the National Heart, Lung, and Blood Institute's Guidelines for the Diagnosis and Management of Asthma.

The Horizon NJ Health Asthma "Breathe Easy" Program can help by teaching the basics of asthma, asthma triggers, treatments, how to check progress, and how to get emergency care. Members will learn how to:

• Know what their asthma triggers are and how to make their home a healthier place.
• Control their asthma with medication.
• Keep track of their progress.

The Asthma "Breathe Easy" Program also includes home visits by visiting nurses and specialist referrals for eligible members. Members will also receive educational information by phone and mail.
