Your Primary Care Provider (PCP) and Primary Care Dentist (PCD)

When you enroll with Horizon NJ Health, you choose a personal doctor or a nurse practitioner, called a Primary Care Provider (PCP). If eligible for dental services, you are also given your own Primary Care Dentist (PCD). Your PCP and PCD are the first sources for health care. Your PCP and PCD are listed on the front of your Horizon NJ Health member ID card. Visit Your PCP and PCD Horizon NJ Health covers PCP visits and eligible dental visits, like well-care check-ups and baseline physicals. You do not need to wait until you are sick to see your doctor or dentist. Your PCP and PCD will make sure you are getting the quality care you deserve. You can help by keeping all of your appointments. Changing Your PCP or PCD You can change your Primary Care Provider or Primary Care Dentist, just call Member Services. - Changes to your PCP will be effective within 10 days
- Changes to your PCD follow this schedule:
- If you ask to change your PCD on or before the 20th of the month, the change is in effect on the first day of the next month
- If the change is requested after the 20th of the month, your new PCD starts on the first day of the second month.
