How Do I Apply?

To get a NJ FamilyCare application, call the State's Health Benefits Coordinator (HBC) toll free at 1-800-701-0710 and provide your name and address. An application will be mailed to your home. You can also downloadthe application or complete the application online at NJ FamilyCare.

Personal assistance with completing the NJ FamilyCare application is available from Horizon NJ Health. Call toll free at 1-800-637-2997. Hearing- and speech-impaired members may use their TDD/TTY equipment or service to call Horizon NJ Health's TDD/TTY toll-free line at 1-800-654-5505.
