To apply for NJ FamilyCare Advantage, download the Horizon NJ Health NJ FamilyCare Advantage application and mail it with the necessary supporting documents to:

Horizon NJ Health NJ FamilyCare Advantage Application

You must include the following items with your application for the enrollment process:

  • The most recent pay stub for each working person in your household. If you are self-employed, or the owner of a business, submit a signed copy of the most recent IRS 1040 Form (including all appropriate Schedules). You may also submit your last profit and loss statement.
  • Proof that you are a resident of New Jersey, such as a utility bill or copy of a Driver License.
  • Documentation that your child(ren) has been without health insurance for six months or more, or that your child(ren)has lost NJ FamilyCare/Medicaid eligibility.
  • Your initial premium payment must be included in your application package.
  • Do not forget to select a Primary Care Provider for each of your children!

Can I get a printed application?

Applications can be mailed to a requestor's home by calling Horizon NJ Health at 1-800-637-2997.

If I need help filling out the application, what should I do?

Horizon NJ Health can help you complete the application. You can call us toll free at 1-800-637-2997 from 8:00 a.m. until 7:00 p.m. Monday through Friday.

What is a Certificate of Credible Coverage? If I don't have it, can I still apply?

A Certificate of Credible Coverage provides evidence of prior insurance coverage and it is supplied by the health plan that previously covered your child. This document includes the dates covered by that plan. You will need to attach a copy of this letter with your application.

How long will the application process take?

Once we receive your application, it will take approximately two weeks to process. If we require additional information, we will contact you. We will notify you by mail if your child has been accepted for membership or if their application is denied. If coverage is denied, a reason will be given.

Do I need to select a Primary Care Provider (PCP)?

Yes! Each member should select a PCP from the Horizon NJ Health provider network. PCPs are listed in the Provider Directory. Copies are available on the Horizon NJ Health Web site or you can call Horizon NJ Health's NJ FamilyCare Outreach Center at 1-800-637-2997 for help in finding a PCP in your area. If a PCP is not selected on the application, we will assign your child a PCP based on your neighborhood. You can change your child's PCP at any time.
