Recall of EpiPen and EpiPen Jr Auto-Injectors (0.3 mg and 0.15 mg)

On March 31, 2017, Mylan issued a voluntary recall of 13 lots of EpiPen and EpiPen Jr Auto-Injectors (0.3 mg and 0.15 mg). It is possible that these devices may have a defective part that may cause device not to work properly. The lots were distributed between December 17, 2015 and July 1, 2016.

Please see below for the recalled lots. Check the lot number on your carton or device to see if your EpiPen Auto-injector is affected by the recall and note the following:

  • If you are impacted by this recall, you can contact Stericycle at 1-877-650-3494 (TTY 711) to obtain a voucher code for your FREE replacement product.
  • Stericycle also will provide you with a pre-paid return package to ship the product back to Stericycle.
  • Visit your pharmacy with your voucher information to redeem your free replacement.
  • Send your recalled product to Stericycle. Do not return any devices affected by the recall until you have your replacement in hand.
  • Stericycle’s hours of operation are Monday through Friday from 8 a.m. to 10 p.m., Eastern Time, and Saturday and Sunday, 8 a.m. to 5 p.m., Eastern Time.
