It is important to establish a dental home for you and your loved ones. Your Pediatrician or Primary Care Provider (PCP) can help you establish a dental home, especially if you have children less than 3 years old.
• Your PCP should be performing an oral screening during your child’s and your yearly physical if you or your children are under 21 years of age. • The oral screenings help determine the health of your teeth and gums as well as your tonsils and throat. • After the oral screening, your PCP should refer you to a dentist for a complete oral examination and to establish a dental home. It is important to see the dentist even if you think nothing is wrong. You do not need a referral form to see the dentist.
It is then up to you to keep the appointment with the dentist. This will help ensure that you and your loved ones stay on the road to good oral and overall health. If you need help finding a dentist, please call Horizon NJ Health Member Services toll free at 1-877-7NJ-HEALTH (1-800-765-4325).
A dental home is the place where one dental practice cares for and oversees all your oral health needs. Schedule a dental check-up every six months to help keep your family healthy.
