Physician and Health Care Hotline: 1-800-682-9091
Physicians may call our Physician and Health Care Hotline 24 hours a day, seven days a week with questions and/or concerns regarding Horizon NJ Health, a member, or other issues. Our representatives will be happy to assist you.

Utilization Management: 1-800-682-9094
If you are a Physician seeking prior authorization or are in need of other assistance concerning a member's treatment, call our Utilization Management Hotline (8:00 a.m. to 8:00 p.m.) seven days a week.

Case Management: 1-800-682-9094
Physicians may call the Case Management Hotline in cases of emergency. If you are a physician who needs to speak with a Case Manager regarding an emergency with a member's care, please call our Case Management Hotline (8:00 a.m. to 8:00 p.m.) seven days a week.

Social Case Management: 1-800-656-3729
Members in need of special assistance with issues, such as housing, food, substance abuse, domestic violence, etc., can call our Social Case Management Hotline. Our team of certified/licensed social workers may be able to help. If you are a physician, family member, friend, or someone concerned about a member in need of special services, please call our Social Case Management Hotline.
