Claims Filing Date of Service

Effective April 19, 2008, Horizon NJ Health requires that any provider billing for Outpatient service utilizing a UB-04 or an electronic transaction must ensure that the dates of service are within the time period indicated in the "Statement Covers Period" on the claim. If the date of service is outside the dates placed in the "From/Through" field, the claim will be denied. If you have any questions, please contact your Provider Representative.
