New Medical Appeal Request Form

When authorization requests are denied, facility and provider denial notification letters will now include a new form known as a Medical Appeal Request Form. When the provider receives this form - which is optional to use - it will already contain member information, authorization details and the correct PO Box to which to send the appeal. This will help ensure the appeal is directed to the appropriate department to allow for prompt processing. This form may be used in place of other appeal letters you may have used in the past. You will still be required to furnish clinical documents so there will be a full and thorough review of your appeal.
