Horizon NJ Health has an extensive network of Primary Care Physicians (PCPs), dentists, specialists, pharmacies, and hospitals, all providing access to health care for our members. Benefits and services that our members receive through Medicaid and NJ FamilyCare programs include well-child visits, prenatal care, routine care, vision care, dental care, prescription and nonprescription medications, special needs case management, transportation, and much more. Our health programs are specially designed to help Horizon NJ Health members get the most out of their health care benefits through programs like the Asthma "Breathe Easy" Program and Mom's GEMS Prenatal Program. Members have access to health care 24 hours a day, seven days a week, through our network of PCPs.

Our ability to focus on the unique needs of our membership affords us the advantage of supporting initiatives that will enhance the distinct outreach, health education, and prevention programs by targeting issues to meet the specific needs of the populations served by Horizon NJ Health.

Horizon NJ Health has been awarded full URAC accreditation and has been nationally recognized by the American Association of Health Plans for its innovative disease management programs. We have established community partnerships, including health outreach and prevention programs for the publicly insured members, while fostering successful partnerships with our Physician Network.
